Modern surgical technology makes it possible to construct a natural-looking breast after mastectomy (breast removal) for cancer or other diseases. The procedure is commonly begun and sometimes completed immediately following mastectomy, so that the patient wakes with a new breast mound instead of no breast at all. Alternatively, reconstruction may begin years after mastectomy. Insurance companies cover reconstruction following breast cancer surgery, and Congress has legislated to make coverage mandatory.  Also, in the state of California, procedures on the contralateral breast for symmetry are mandated for insurance coverage as well.

Most women choose and are eligible for immediate reconstruction.There are several ways to reconstruct the breast, both with and without implants; your breast surgeon and plastic surgeon should work together with you in deciding which approach is best for you.

The most common used method is using tissue expanders and permanent breast implants.  A breast expander is placed in chest at the time of the mastectomy.  With modern advances to both breast implants and the use of dermal matrix, we have gone from only offering under the muscle reconstruction to now mostly performing pre-pectoral reconstruction.  In this situation, the implant based reconstruction is placed on top of the chest muscles.  This significantly reduces the pain of surgery and eliminates cutting and stretching the pectoralis muscle.  This allows reconstruction to be a much more comfortable experience for women with no chest to the biomechanics of the chest wall muscles.  As an outpatient, the expander is gradually expanded to the desired size before a second outpatient surgery is required to replace the tissue expander with the desired implant.  At the time of the 2nd procedure, the cosmetic outcome is optimized by any combination of scar removal, skin removal, or fat grafting.
